The principles and importance of the spino-pelvic balance is demonstrated. The clinical appearance, anatomy, physical examination and surgery indication in a high grade L5-S1 isthmic spondylolisthesis is explained. The surgical technique and tricks in reduction and fusion of the high grade spondylolisthesis is demonstrated.
